I believe the first one could go in the fail thread on dtr. The guy is lucky he is not dead.
As for the second, very unfortunate, hate to hear something like that.

According to the media it is the gun's fault. There is no responsibility for a person to be intelligent enough to research something they are to do.
There have been several actors killed, Brandon Lee is one, there was the star of a show called Cover Up that is another that comes to mind. All from ignorance and not knowing safety.
